SUMMARY
The Child and Family Service Provider covers several services in the
community based on the needs of the child and family. The primary
service of this is position is to provide Psycho-Social Rehabilitation
(PSR) and HCBS Waiver Services. The services are goal-directed
supports and solution-focused interventions intended to address
challenges associated with a behavioral health need and to achieve
identified goals or objectives as set forth in the child’s treatment
plan. PSR and Waiver Services are a face- to-face intervention with
the child/youth in the community or in the home. The main component of
PSR is to teach skills that improve functional impairments. Other
services could include Respite, Community Self-Advocacy, and Care
Giver Family Supports.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Plans and implements PSR and Waiver Service activities in accordance
to the child’s individualized service plan, using creativity and
utilizing community resources.
Builds a strong therapeutic rapport with the family that promotes a
collaborative and cooperative working relationship.
Provides Psycho-Social Rehabilitation, Community Self Advocacy,
Caregiver Supports and Respite Services.
Maintains familiarity with the child’s service plan, evaluates
child’s progress in collaboration with Care Manager, Therapist,
Doctors and other service providers.
Identifies activities that support the targeted developmental tasks,
strengths, and/or talents
Receives information about the child and family from the Care Manager
and Therapist including updates on the service plan and safety plans.
Accurately maintains sufficient documentation of goal oriented
progress notes.
Maintains client confidentiality and boundaries at all times
Follows and maintains agency policies and procedures in all program areas.
Attends appropriate training and in-service seminars in accordance
with agency requirements.

EDUCATION and/or EXPERIENCE
High School Diploma or GED and 3 years’ experience in mental health,
foster care or addiction services.
