The primary service of this is position is to provide HCBS Waiver
Service of hourly Respite. The service is goal-directed support and
solution-focused intended to assist in addressing challenges
associated with a behavioral health need and to achieve identified
goals or objectives as set forth in the child’s treatment plan. Waiver
Respite Services are a face- to-face intervention with the child/youth
in the community. The main component of respite is to provide a break
to the child and family in order to improve functional impairments.
Other services could include Respite and Care Giver Family Supports.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Plans and implements Waiver Respite Service activities in accordance
to the child’s individualized service plan, using creativity and
utilizing community resources.
Builds a strong therapeutic rapport with the family that promotes a
collaborative and cooperative working relationship.
Provides Respite Services to clients.

Maintains familiarity with the child’s service plan, evaluates
child’s progress in collaboration with Care Manager, Therapist,
Doctors and other service providers.
Identifies activities that support the targeted developmental tasks,
strengths, and/or talents
Receives information about the child and family from the Care Manager
and Therapist including updates on the service plan and safety plans.
Accurately maintains sufficient documentation of goal oriented
progress notes.
Maintains client confidentiality and boundaries at all times
Follows and maintains agency policies and procedures in all program areas.
Attends appropriate training and in-service seminars in accordance
with agency requirements.
Other job related duties as assigned

EDUCATION and/or EXPERIENCE
High School Diploma with some experience working with children.
