Job Description:

SUMMARY

The Registered Nurse functions as a member of a mobile, multi-disciplinary mental health treatment team serving individuals with severe mental illness in their homes and communities in Jefferson and Lewis Counties. The team provides a full range of clinical treatment, psychosocial rehabilitation, and community support services designed to promote recovery by improving psychiatric symptoms, preventing relapse, teaching skills, providing direct assistance and securing community resources necessary for successful functioning in work, school, home and social relationships.

This position involves responsibility for performing a variety of professional nursing tasks for clients of the ACT Team. The work is performed in collaboration with the ACT team under the supervision of the ACT Team Leader; with clinical supervision provided by the Nurse Practitioner. The RN will exercise independent judgment in carrying out the daily work load. Develops and implements intervention protocols for client care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The Registered Nurse is responsible for:

  • Assessing physical health needs
  • Making appropriate referrals to community physicians
  • Providing management and administration of medication in conjunction with the Psychiatrist/Psychiatric Nurse Practitioner
  • Providing a range of treatment, rehabilitation, and support services

Attends morning team meetings.

Provides professional psychiatric nursing services as a member of a multi-disciplinary clinic treatment team.

Completes comprehensive health screenings on all ACT clients.

Records prescriptions in medication records and escribing.

Follows agency policies and procedures in all program areas.

Completes prior authorizations and maintains authorization process binder with current information and formularies for each insurance company serving ACT clients.

Completes required ACT curriculum and training, as well as participates in any additional education required by the agency.

Other job-related duties as assigned.

Experience and Skills:

EDUCATION and/or EXPERIENCE

Possession of a current license and registration as a Professional Nurse in New York State and possession of a current New York State certification as a Registered Nurse with a minimum of 6 months’ psychiatric experience.
