Community Psychiatric Support and Treatment (CPST) includes
goal-directed supports and solution-focused interventions with the
intent to achieve person-centered goals and objectives. This is a
multi-component service that consists of therapeutic interventions
such as clinical counseling and therapy, which assist the individual
in achieving stability and functional improvement. CPST addresses
behavioral health barriers that impact daily living, finances,
housing, education, employment, personal recovery and/or resilience,
family and interpersonal relationships and community participation.
CPST is designed to provide mobile treatment services to individuals
who have difficulty engaging in site-based programs, or who have not
been previously engaged in services, including those who had only
partially benefited from traditional treatment. CPST allows for
delivery of services within a variety of permissible of-site settings
including, but not limited to, community locations where the
individual lives, works, learns, and/or socializes.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The CPST provider will also act as the Licensed Professional of the
Healing Arts (LPHA) and ensure all referrals have proper
authorizations in place, including for other CORE providers in the
tri-county area, as needed.
Assist in developing a referral network in the tri-county area for
CORE services.
Analyze data from various electronic health records or data rich
platforms, such as PSYCKES, looking at quality flag indicators to
address barriers by connecting these individuals to CORE services, if appropriate.
Create and provide educational workshops/informational sessions for
TLS and other community agencies/committees to further promote CORE
and Behavioral Health Home and Community Based Services (BH HCBS).
Notify an enrollee’s Managed Care Organization (MCO) within three
business days after the first date of initiating a new CORE service
and conduct intake and evaluation sessions within 30 days of the
initial visit or the first five visits, whichever occurs later.
Develop the initial individual service plan (ISP) and ISP reviews
every six months or whenever there is a significant life changing event.
Identify activities that support the targeted developmental tasks,
strengths, and/or talents.
Implement identified/approved service activities in accordance to the
individualized service plan, using creativity and utilizing community resources.
Build a strong therapeutic rapport with the individual that promotes
a collaborative and cooperative working relationship.
Maintain familiarity with the ISP and evaluate it in collaboration
with other service providers.
Receive information about the individual from the doctor and
therapist including updates on the service plan and safety plans.
Accurately maintain sufficient documentation of goal-oriented
billable and non-billable progress notes.
Maintain client confidentiality and boundaries at all times.
Follow and maintain agency policies and procedures in all program areas.
Attend appropriate training and in-service seminars in accordance
with agency requirements.
Supervise and support the Psychosocial Rehabilitation (PSR) and
Family Support and Training (FST) providers.
Conduct chart audits and other quality assurance measures.
Complete reports for the state and/or TLS as required.
Other job related duties as assigned.
Management has the right to add or change the duties of this

The CPST provider is defined as a clinical practitioner possessing a
license or a permit from the New York State Education Department who
is qualified by credentials, training, and experience to provide
direct services related to the treatment of mental illness and
substance use disorders and shall include the following:
- Physician: An individual who is currently licensed or possesses a
permit to practice medicine issued by the New York State Education Department;
- Psychiatrist: An individual who is currently licensed or possesses
a permit to practice medicine issued by the New York State Education
Department and who is either a diplomate of the American Board of
Psychiatry and Neurology or is eligible to be certified by such
Board or is certified by the American Osteopathic Board of Neurology
and Psychiatry or is eligible to be certified by such Board;
- Physician assistant: An individual who is currently registered or
possesses a permit to practice as a physician assistant issued by
the New York State Education Department;
- Nurse practitioner: An individual who is currently certified or
possesses a permit to practice as a nurse practitioner issued by the
New York State Education Department;
- Psychiatric nurse practitioner: An individual who is currently
certified or possesses a permit to practice as a nurse practitioner
with an approved specialty area of psychiatry issued by the New York
State Education Department;
- Registered nurse: An individual who is currently licensed or
possesses a permit to practice as a registered professional nurse
issued by the New York State Education Department;
- Psychologist: An individual who is currently licensed or possesses
a permit to practice as a psychologist issued by the New York State
Education Department;
- Social worker: An individual who is either currently licensed to
practice as a licensed master social worker (LMSW) or as a licensed
clinical social worker (LCSW) issued by the New York State Education Department;
- Mental health counselor: An individual who is currently licensed
or possesses a permit to practice as a mental health counselor
issued by the New York State Education Department;
- Marriage and family therapist: An individual who is currently
licensed or possesses a permit to practice as a marriage and family
therapist issued by the New York State Education Department;
- Psychoanalyst: An individual who is currently licensed or
possesses a permit to practice as a psychoanalyst issued by the New
York State Education Department; and,
Creative arts therapist: An individual who is currently licensed or
possesses a permit to practice as a creative arts therapist issued by
the New York State Education Department
